29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about November 24, 2015, the employer failed to ensure employees cleaning the Glass Roll Press model number MHM03500416, serial number 10253, were trained as authorized employee's in lockout/tagout. An employee's arm was caught in the roller assembly when another employee turned power back on. The Vista Window Company, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(7), which was contained in OSHA inspection number 1003030, citation number 01, item number 005 and was affirmed as a final order on May 7, 2015, with respect to a workplace located at 1701 Henn Parkway SW, Warren, Ohio 44481.
