29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: On or about July 30, 2015, the employer failed to ensure employees that have to access the side panel of the M-Series Nut Tappers to remove jams verified no hazardous energy sources were present prior to beginning work. Telefast Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910147(c)(4)(i), which was contained in OSHA inspection number 990085, citation number 01, item number 001 and was affirmed as a final order on October 30, 2014, with respect to a workplace located at 777 West Bagley Road, Berea, Ohio 44017.
