29 CFR 1904.7(b)(4): An injury or illness involving restricted work days or job transfer, was not recorded on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and an entry of the number of restricted or transferred days in the restricted workdays column: World Kitchen, LLC, on or about 9/19/14: The employer did not record the following workplace injuries and illnesses correctly on the OSHA 300 Log for calendar year 2014. a) Case No. 27, on or about 10/1/14 an employee had a left shoulder strain from clearing 471 line of 418s, the case resulted in 13 restricted work days and was recorded with only 8 restricted work days and no check mark in Column I. b) Case No. 31, on or about 10/19/14 an employee had tendinitis in the right elbow from picking up stack 2213 from line and putting them on cart and pushing the cart, the case resulted in 11 restricted work days and was recorded with a check mark in Column J when the check mark should have been in Column I. World Kitchen, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.4(a), which was contained in OSHA inspection number 116211335, citation number 1, item number 1 and was affirmed as a final order on 7/28/10, with respect to a workplace located at 1200 South Antrim Way, Greencastle, PA 17225.
