29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a) Firing Range, Dip Tank, and Polishing Areas: Henry RAC Holding Corp., 59 East 1st Street, Bayonne, NJ 07002 The employer did not provide medical evaluations for employees exposed to lead and corrosives such as but not limited to sodium hydroxide found in oxiblak. During work operations, employees are required to use half mask negative pressure respirators equipped with HEPA filters. Conditions noted on or about 09/21/2012 and 12/14/2012. Henry RAC Holding Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(e)(1), which was contained in OSHA inspection number 312480676, citaiton number 1, item number 3b and was affirmed as a final order on 11/15/2008, with respect to a workplace located at 110 8th Street, Brooklyn, NY 11215.
