29 CFR 1926.1432(b)(2): The lift director did not review the multiple crane lift plan in a meeting with all workers who were involved with the operation. On or about July 5, 2012, a lift director did not review the multiple crane lift plan in a meeting with all employees who were involved in the placement of bridge girders over Lake Butte des Morts, Bridge B-70-266. Employees were thereby exposed to the hazards associated with crane and load failures.
