29 CFR 1904.29(b)(2): The employer did not complete an OSHA 301 Incident Report form, or an equivalent form, for each recordable injury or illness entered on the OSHA 300 Log. At the inspection site, for calendar years 2009 through 2011, the OSHA 301 Incident Reports or substitute forms were incomplete and not maintained at the detail required by the regulation for each case on the OSHA 300 Logs. a) For calendar year 2009, case# 396414 did not have an incident report completed. Also, case# 400476 states that medical treatment was declined in line 6, even though it was recorded on the OSHA 300 Log. And case# 40476 is missing information for line 17, what object or substance directly harmed the employee. b) For calendar year 2010, cases# 53473, 55778, and 55880 are missing information for lines 7 (where was treatment given) and 17 (what object or substance directly harmed the employee). Case# 56442 was missing information for line 17 (what object or substance directly harmed the employee). c) For calendar year 2011, cases# 1599241, 156221, 1609751, and 1676431 are missing information in various lines of the form. Case# 1599241 is dated 5/25/2011 even though the Log shows the date as 8/23/2011. Also, the case recorded on 11/3/2011 is missing a case#. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the OSHA 301 or equivalent forms are complete and accurate.
