29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) 1 Eastern Rd, Kearny NJ A maintenance employee shut down dryer #4 to unjam linens without affixing locks. Violation occurred on or about 1/18/2025 b) 1 Eastern Rd, Kearny NJ Maintenance employees replaced a motor in Ironer #1 without affixing locks to all the identified disconnects identified in the procedures. Violation occurred on or about 1/24/2025
