29 CFR 1926.1408(a)(2)(iii)(A):The line's voltage and the minimum approach distance permitted under Table A (see 29 CFR 1926.1408) were not determined by the employer. Location: 556 Main Street Falmouth Lyberty Green Jobsite On or about October 30, 2024, employees were exposed to hazards associated with electric shock when a steel beam was permitted to get closer to energized power distribution lines than permitted in Table A (in this instance 10 feet). Measurements taken during the inspection and an estimate of the height of the bean in the ironworkers hand puts the beam 5 to 44.28 inches from the distribution line, well withing the 10 foot approach distance.
