Text For Citation: 02 Item/Group: 001B Hazard:
29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about July 23, 2024, the employer did not ensure employees working as pail line mechanic(s) engaged in service and maintenance activities of pail line turnover #2 machine, also known as the turnover 11.25 spin flange machine, applied and followed the established energy control procedure by; turning off or shutting down the machine [per 1910.147(d)(2)], isolating the machine electrical and pneumatic energy sources [per 1910.147(d)(3)], affixing lockout devices [per 1910.147(d)(4)(i)], or verifying de-energization and isolation had been accomplished [per 1910.147(d)(6)] prior to performing service and maintenance activities. Service and maintenance activities conducted by pail line mechanic employees included, inspecting and adjusting the drive chain and sprocket of the turnover #2 machine, unjamming pails at the machine, and performing lubrication and other preventative maintenance and service activities such as, replacing drive chains. Cleveland Steel Container Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d) which was contained in OSHA inspection number 1601640, citation number 1, item number 1b, and was affirmed as a final order on October 21, 2022, with respect to a workplace located at 10048 Aurora Hudson Road, Streetsboro, Ohio.
