29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval: Location: Storage Area On or about 06/24/2024, employees were using a powered industrial truck with a self made platform that was not designed by manufacturer.
