29 CFR 1910.1030(f)(2)(iv): The employer did not ensure that employees who declined to accept the hepatitis B vaccination offered by the employer signed the statement in appendix A: a) CPRD Area: On or about May 9, 2024, the employer did not ensure that the employee signed the declination statement when the employee declined the hepatitis B vaccination when first offered to them by the employer. The employer did not have the employee sign a statement of declination until May 10, 2024.
