29 CFR 1910.184(e)(9)(i): Alloy steel chain sling(s) with cracked or deformed master link(s), coupling link(s), or other component(s) were not removed from service: a) On or about 04/22/2024 at the site located at 3000 Milton Ave., Solvay, NY: Employees exposed to struck by hazards when utilizing an alloy chain sling on Hoist-2 Maintenance Shop (Forklift Repair) underhung lifting device to lift a motor for repair weighing approximately 250 lbs. The alloy chain sling had multiple links that were deformed motor was approximately 6 inches off the ground.
