29 CFR 1904.7(b)(3): When an injury or illness involved one or more days away from work, the employer did not record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days away column. a) 3008 Penns Valley Pike, Centre Hall, PA 16828 - On or about October 25, 2023, an employee tripped on the stairs and suffered a fractured wrist (Case #HFCH20-2300012) resulting in days away from work and the 2023 OSHA Form 300 Log of Work-Related Injuries and Illnesses did not include a check mark in Column H for days away and did not include the number of days away in Column L. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
