Text For Citation: 02 Item/Group: 001B Hazard:
29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about July 14, 2023, the employer did not ensure employees performing service and maintenance activities on the Dake Bow 2 Vertical Press Machine applied energy controls by; turning off or shutting down the machine [per 1910.147(d)(2)], isolating the electrical and hydraulic energy sources [(d)(3)], affixing lockout device(s) to the energy isolation devices [(d)(4)(i)], releasing the stored hydraulic energy [(d)(5)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to setting up dies in the press. Conn-Selmer, Inc., was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(d), which was contained in OSHA inspection 1345036, Citation 1, Item 2 and was affirmed as a final order on February 27, 2019, with respect to a workplace located at 34199 Curtis Blvd, Eastlake, Ohio.
