29 CFR 1910.140(c)(12): Anchorages used to attach to personal fall protection equipment must be independent of any anchorage used to suspend employees or platforms on which employees work. Anchorages used to attach to personal fall protection equipment on mobile work platforms on powered industrial trucks must be attached to an overhead member of the platform, at a point located above and near the center of the platform. A) On or about November 1, 2022, the employer did not ensure that the anchor points on the Crown order pickers SP Series: serial 10331907, Crown Order Picker SP Series: serial 10331908, and Crown Order Picker SP Series: serial 10331909 were independent to suspend employees or platforms on which the employees worked. Two employees were working off of the same anchor point, the bar across the work rest.
