29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a.) Dimensional Metals, Inc., at the jobsite located at 58 Klema Dr. North, Reynoldsburg, Ohio 43068: On and or/preceding March 15, 2022, the employer failed to provide authorized employee training to employees engaging in servicing and maintenance activities, such as but not limited to lubricating and cleaning rollers on the flush panel roll former (model: CA-84-60-18-L-SP S/N: 19685.00). The employees performed these servicing and maintenance activities without receiving authorized employee level lockout/tagout training, thereby exposing employee to struck-by, caught-in, and amputation hazards. b.) Dimensional Metals, Inc., at the jobsite located at 58 Klema Dr. North, Reynoldsburg, Ohio 43068: On and or/preceding March 15, 2022, the employer failed to provide authorized employee training to employees engaging in servicing and maintenance activities, such as but not limited to cleaning rollers on the Forstner CTL machine (model: RM-TSS S/N: 104887). The employees performed these servicing and maintenance activities without receiving authorized employee level lockout/tagout training, thereby exposing employee to struck-by, caught-in, and amputation hazards. c.) Dimensional Metals, Inc., at the jobsite located at 58 Klema Dr. North, Reynoldsburg, Ohio 43068: On and or/preceding March 15, 2022, the employer failed to provide authorized employee training to employees engaging in servicing and maintenance activities, such as but not limited to cleaning, setting up and adjusting on the Schlebach roll former (model: SW25 S/N: SWI18-M1019). The employees performed these servicing and maintenance activities without receiving authorized employee level lockout/tagout training, thereby exposing employee to struck-by, caught-in, and amputation hazards. Dimensional Metals, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1511714, citation number 1, item number1a and was affirmed as a final order on August 8, 2021, with respect to a workplace located at 58 Klema Dr. North, Reynoldsburg, Ohio 43068.
