29 CFR 1910.147(d): The established procedure for the application of energy control (the energy control procedures) did not cover the following elements or actions and was not done in sequence as required by 29 CFR �� 1910.147(d)(1) through (d)(6): a) Radial sheet metal crimping machine - The attempted adjustment of the pneumatic clamping mechanism and related mechanical parts resulted in the inadvertent release of these hazardous energies because the company's procedure for shutdown was inadequate. Specifically, the procedure lacked specific steps to relieve the hazardous pneumatic and mechanical energy of the radial sheet metal crimping machine prior to the equipment being serviced or maintained. As a result the following control measure elements and actions could not sequentially be performed in accordance with the established company procedure: 1. Employees were not knowledgeable about the pneumatic energy present and the means to control the energy as required by �1910.147(d)(1). For example, the operators were not aware of the appropriate pneumatic energy isolation measures as they were not identified in the company's energy control procedure. 2. An orderly shutdown to avoid increased hazards, as required by 1910.147(d)(2), was not performed because the company's general procedure was not completely effective or lacked identification of all the hazardous energies present. 3. All energy isolating devices that were needed to control the hazardous energy for the crimping machine, as required by 1910.147(d)(3), were not operated in such a manner as to isolate the equipment from the energy sources. Therefore, LOTO devices could not be affixed in accordance with �1910.147(d)(4). 4. Residual energy was not relieved or otherwise rendered safe in accordance with �1910.147(d)(5)(i). Lukjan Metal Products North Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1304483, citation number 1, item number 2(a) and was affirmed as a final order on October 16, 2018, with respect to a workplace located at 780 River Ave. S., Prairie Farm, WI 54762 .
