29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. At rear of warehouse located at 1496 Mt. Zion Road, Morrow, GA 30260: On or about 07/20/2021, the employer did not report the in-patient hospitalization of an employee who was injured from a fall suffered in a scaffold incident at approximately 2:00 PM on 07/19/2021. The employer reported a fatality on 07/23/2021 at 4:15 PM after the injured employee died from his injuries suffered from the fall in the hospital on 07/20/2021 at 9:30 PM.
