29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): Production Floor: Line 2, Side B, Pasta Technologies Group Bi-Colored Filled Pasta Line: On, or about, March 22, 2021, an employee was severely injured when he was pulled into the top of the front hopper while performing a quality inspection. The employer had not implemented procedures to protect employees during quality inspections when machines were released from lockout and turned on to check for any material that may have been missed during the cleaning procedure. Carla's Pasta, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.147(c)(4)(i)], which was contained in OSHA Inspection Number 1194248, Citation Number 1, Item Number 6 and was affirmed as a Final Order on March 28, 2017, with respect to a workplace located at South Windsor, Connecticut.
