Location: Oakland Grove Health Care Center, Woonsocket, RI On or about 8/13/2020, the employer did not provide a fit test to all employees who were required to wear N95 respirators. The employer required employees to wear N95 filtering face piece respirators to protect against the SARS-CoV-2 virus while providing care to suspected or confirmed positive COVID-19 patients and residents.
