29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) does not cover the actions listed in and is not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Boomerang Rubber, Inc., located at 105 Dinsmore Street, Botkins, OH 45306: The employer does not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, are performed. Employees do not de-energize and lockout all hazardous energy sources of the Press 17 (built in house), located in the Building 3 Press Room, prior to conducting activities such as, but not limited to, rebuilding the shuttle (replacing hooks with magnets) on the West side of the press. Employees are exposed to struck-by and caught-in-between hazards that can result in serious injuries while performing this task. This violation most recently occurred on or about January 17, 2020, at the above address. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] are not implemented to protect employees from struck-by and caught-in-between hazards while employees are conducting activities such as, but not limited to, rebuilding the shuttle (replacing hooks with magnets) on the West side of the Press 17. Boomerang Rubber, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standards (29 CFR 1910.147(d)(2)), which was contained in OSHA Inspection # 1197736, Citation number 1, Items number 1, and was affirmed as a final order on 03/09/2017, with respect to a workplace located at 105 Dinsmore Street, Botkins, OH 45306. Boomerang Rubber, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standards (29 CFR 1910.147(d)(4)), which was contained in OSHA Inspection # 107507, Citation number 1, Item number 3c, and was affirmed as a final order on 12/19/2012, with respect to a workplace located at 105 Dinsmore Street, Botkins, OH 45306.
