29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a) Jobsite located at 3320 South 20th Street, Philadelphia, PA: The employer failed to instruct the employees who were working in an approximately 10 foot deep 10 foot long and 8 foot wide excavation without shoring, exposed reinforced steel, and unguarded machines a way to avoid the unsafe work conditions. Condition was observed on or about 5 SEP 2019. Solid Concrete Walls, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1) which was contained in OSHA inspection number 1371372, citation number 1, item number 1/b, and was affirmed as a final order on 30 JUL 2019, with respect to a workplace located at 2120 East York Street, Philadelphia.
