29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: a. Site - An employee was working from a makeshift scaffold which consisted of two sheets of plywood resting atop the forks of an all-terrain forklift which was lifted to approximately 14 feet above the ground and was not secured to the forklift. Clancy Builders, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.602(c)(1)(viii)(a), which was contained in OSHA inspection number 898809, citation number 001, item number 004B, and was affirmed as a final order on 2/27/2014, with respect to a workplace located at 10 Woodview Way, Manchester, NH 03102.
