29 CFR 1926.451(g)(3): Personal fall arrest systems used on scaffolds were not attached by lanyard to a vertical lifeline, horizontal lifeline, or scaffold structural member. a) 1000 Ferry Road, Wilmington, Delaware - On or about January 10th, 2019, the employer failed to allow employees to wear fall protection connected to a vertical, horizontal, or scaffold member where the mast and scaffold working level were rated for over five thousand (5,000) pounds required for a personal fall arrest system.
