29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. (a) North building - On or before November 28, 2018 the employer did not affix a lockout or tagout device exposing employees to amputation hazards from energy sources such as but not limited to electrical and kinetic energy while performing tasks including but not limited to start-up, maintenance, servicing, and jam clearing of the Farrell Compound Mixer.
