Rockbranch Ironworks, Coulterville, IL: On or about 09/20/2018, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out or incorrectly filled for the calendar year 2018. On or about 09/20/2018, a fatality occurred when an employee was electrocuted while performing welding operations at a jobsite and an OSHA 301 was not filled out.
