29 CFR 1910.147(c)(2)(ii): An energy isolating device that was capable of being locked out, did not utilize a lockout device, which rendered the machine or equipment inoperative: a) On or about June 26, 2018, in the Heat Treat Department, employees were required to work in and around the large heavy quench tank and its circulating water pump's energy isolating device (electrical disconnect) had a string and tag in place of a lock.
