29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Major Metals Company, located at 800 Kochheiser Road, Mansfield, OH 44904: On or about February 8, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d) and required for the control of the hazardous energy sources such as, but not limited to, hydraulic were followed before employees conducted activities such as, but not limited to, adjusting cooling lines located near the machines point of operation on the equipment such as, but not limited to, the Haven 3 tube cut off machine (Model # 872, Serial # 3020) located in Mill 4 Building. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from amputation hazards while employees conduct activities such as, but not limited to, adjusting cooling lines located near the machines point of operation on the equipment such as, but not limited to, the Haven 3 tube cut off machine (Model # 872, Serial # 3020) located in Mill 4 Building. Major Metals Company was previously cited for a violation of an equivalent Occupational Safety and Health Standard (1910.147(D)(2)), which was contained in OSHA Inspection # 956027, Citation number 2, Item number 2B, and was affirmed as a final order on July 29, 2014, with respect to a workplace located at 844 Kochheiser Road, Mansfield, OH 44904.
