29 CFR 1910.147(c)(7)(iv): The employer did not certify that employee training had been accomplished and was being kept up to date: (a) On December 7, 2017 and times prior thereto, board edger operators perform lock out tag out of board edger without being LOTO trained. (b) On December 7, 2017 and times prior thereto, maintenance employees perform lock out tag out of board edger without being LOTO trained. Evergreen Mill, LLC dba Clearwater Forest Industries was previously cited at Evergreen Mill, LLC dba Clearwater Forest Industries, for a REPEAT violation of this occupational safety and health standard, 29 CFR 1910.147(c)(7)(iv), which was contained in OSHA inspection number 984232, citation number 2, item number 2c, and was affirmed as a final order on December 12, 2014, with respect to a workplace located at 4686 Hwy 13 S, Kooskia, ID 83539.
