29 CFR 1910.147(f)(1)(i): When lock-out or tag-out devices were temporarily removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions in 29 CFR 1910.147(f)(1)(i) through (f)(1)(v) were not followed. On or about 4 August 2017, and at times prior thereto, the employer did not ensure that procedures were followed for establishing a sequence of actions to be taken when testing the HMC Bagger when knife blade adjustments are made, exposing the employees to an amputation hazard on the HMC Bagger. John B. Sanfilippo & Son, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(4)(ii), which was contained in OSHA inspection number 1022069, citation number 01, item number 001/b and was affirmed as a final order on 06/03/2015, with respect to a workplace located at 1703 N. Randall Rd., Elgin, IL 60123.
