Text For Citation: 02 Item/Group: 001B Hazard:
29 CFR 1910.147(d): Application of control. The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). On or about 08 May 2017, and on dates prior, prior to starting work on the machines, to include adjusting the timing on the CMD Coreless Winders, employees were not required to apply hazardous energy controls as follows: (a) prepare for shut down by recognizing energy hazards and its controls, (b) turn off and shut down the machines, (c) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (d) affix lockout or Tag-out devices to each energy isolating device, (e) render safe all potentially hazardous stored or residual energy, (f) verify that isolation and de-energization of the machine or equipment have been accomplished. The employer failed to take the necessary steps to ensure that their employees were properly protected against hazards associated with caught in hazards, struck-by hazards, and amputation hazards due to the lack of properly guarding of all production machinery. An employee performing adjustment tasks, had his fingers exposed to caught-in hazards when he used magnets to bypass the safety interlocks to gain access to the machine during operation. Hazardous energy controls (Lockout) were not implemented and performed to protect the employee from the caught-in hazard, likely to result in amputation. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d). Colonial Bag Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1044883, citation number 1, item number 1 and was affirmed as a final order on 09/30/2015, with respect to a workplace located at 205 Fullerton Ave. Carol Stream, IL. 60188.
