29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight capacity of the truck and attachment combination at maximum elevation with load laterally centered: 101 North Main Street, Newark, Illinois, west building shop, counterbalance powered industrial trucks - on or about May 4, 2017, modifications were made to fork attachments that affected trucks' capacities and safe operation and were not marked as required to identify this attachment configuration and show the approximate changes in weight capacities. Holes were drilled in forks to secure a job-made fifth wheel with pins/bolts for towing dump trailers. Employees were exposed to struck-by and/or crushed-by hazards. Louis Dierzen dba Dierzen Sales, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(a)(5), which was contained in OSHA inspection number 1125876, citation number 01, item number 002(b) and was affirmed as a final order on August 29, 2016, with respect to a workplace located at 101 N. Main St., Newark, IL 60541. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
