29 CFR 1910.217(d)(9)(iv): Safety blocks were not used when mechanical power press die(s) were being adjusted or repaired in the press: American Fine Sinter Co., Ltd. - Tiffin, OH: On or about March 29, 2017, the employer did not require the use of safety blocks while employees were changing and adjusting dies in the #AI-80-02 Aida sizing press (s/n 10408-0930). Employees were exposed to amputation injury while performing die change activities. American Fine Sinter Co., Ltd. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.217(d)(9)(iv), which was contained in OSHA inspection number 946925, citation number 1, item number 3 and was affirmed as a final order on February 10, 2014, with respect to a workplace located at 957 N. Maule Rd., Tiffin, OH 44883.
