29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: On or about March 1, 2017, employees required to perform servicing tasks on Line 3, such as cleaning on a daily basis, were exposed to fractures, lacerations, and amputations due to the unexpected startup of equipment. Maintenance employees were isolating and locking out energy sources on behalf of exposed employees and thus they were not afforded a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device during the cleaning operations. Hydrox Chemical Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(f)(3)(i), which was contained in OSHA inspection number 1020313, citation number 1, item number 1(b) and was affirmed as a final order on August 14, 2015, with respect to a workplace located at 825 Tollgate Road, Elgin IL 60123. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
