29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees. Waterbury Location: An employee, as "affected and/or other type" was not provided with training and information in the controlling of potentially hazardous and unexpected releases of energy sources (lockout/tagout) during servicing, maintenance and unjamming of equipment, such as (but not limited to) the Langston Die Cutter machine. On or about 01/30/2017, an employee sustained severe injuries which resulted in an amputation of four fingers of his dominant hand after his hand was pulled into a die cutter machine's rollers while attempting to clear a cardboard jam.
