29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): (a) 1511 Washington Street East: On or about January 10, 2017 and times prior, a protective system was not in place to protect an employee working in a previously excavated, 3 foot 4 inch by 13 foot 9 inch and up to 5 foot 4 inch deep trench to install underground fire service to an adjacent building. Tri-State Pipeline, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.652(a)(1); Requirements for protective systems, which was contained in OSHA inspection number 1029187, citation number 2, item number 3 and was affirmed as a final order on April 27, 2015 with respect to a workplace located at the jobsite in Minford, Ohio.
