29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources. Employees were not provided with lockout/tagout hardware to isolate the energy sources while performing maintenance activities on the Telastacker Conveyor exposing employees to unexpected energization. The Johnson County Pipe was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(5)(ii)(D), which was contained in OSHA inspection number 953849, citation number 1, item number 1 and was affirmed as a final order on July 15, 2014, with a final abatement date of July 17, 2014, with respect to a workplace located at 800 CR 209, Alvarado, Texas.
