29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee does not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: The employer does not ensure that both authorized employees engaged in blade changing activities on the Baker Products Band Resaw affix a personal lock to an energy isolating device or a group lockout device. This violation reportedly occurred on or about October 4, 2016, when an employee was exposed to amputation and laceration hazards while changing the machine's blade. To abate this violation, the employer must ensure that hazardous energy sources are isolated by all employees performing maintenance activities. A&D Wood Products, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.147(c)(1), which was contained in OSHA inspection number 108854, citation number 1, item number 3 and was affirmed as a final order on May 3, 2012, with respect to a workplace located at 112 Ward St., Elida, Ohio 45807.
