29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : The employer did not obtain new annual audiograms for employees with the following exposures: a) Chrome mill: A mill operator was exposed to continuous noise levels in excess of the allowable 8-hour time-weighted average sound level (Action Level). The employee's dosimeter dBA level was 129.8% of the permissible level (90 dBA) and was equivalent to approximately 91.8 dBA. The sampling was performed for 406 minutes during one shift on 6/2/16. Zero exposure was assumed for the unsampled period of time. b) Mill number 3: A mill operator was exposed to continuous noise levels in excess of the allowable 8-hour time-weighted average sound level (Action Level). The employee's dosimeter dBA level was 93.4% of the permissible level (90 dBA) and was equivalent to approximately 89.5 dBA. The sampling was performed for 480 minutes during one shift on 6/2/16. Zero exposure was assumed for the unsampled period of time. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
