29 CFR 1904.39(a)(2): The employer did not report within twenty-four (24) hours, the in-patient hospitalization of one or more employees, or an employee's amputation, or loss of an eye as a result of a work related incident to OSHA. On or about November 9, 2015, and employee was injured at the jobsite and the employer did not report the work related in-patient hospitalization prior to the elapse of the 24 hour reporting period of the incident.
