29 CFR 1904.39(a)(2): The employer did not report an inpatient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On June 10, 2015 at the worksite located at 1850 E. Huntsville Road, Fayetteville, Arkansas, an employee sustained an illness resulting in inpatient hospitalization. The employer failed to report the incident to OSHA within the twenty-four hour time period as required.
