29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: P&W Painting Contractors, Inc. - Toledo, Ohio: On or about September 30, 2014, the employer failed to assure that the ladderway floor opening, located on crane's access platform, was guarded to protect employees from fall hazards of approximately 14.5 feet. P&W Painting Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(c)(1), which was contained in OSHA inspection number 313788044, citation number 1, item number 1a and was affirmed as a final order on August 24, 2012, with respect to a workplace located at 26 Main St., Toledo, Ohio 43605.
