29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform: JOB SITE: 200 Harding Avenue, Maryland Heights, MO 63043, at this work site, a worker was observed on a tubular welded scaffold actively working from the working platform and midrails were not installed between the top edge of the guardrail system and the scaffold platform. The worker was exposed to a fall hazard. RENAISSANCE ROOFING, INC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(iii) which was contained in OSHA inspection number 313112005, citation number 1, item number 2, issued on 7/13/2009, and was affirmed as a final order on 8/05/2009, with respect to a workplace located at 3438 Pestalozzi Street, Saint Louis, MO 63118.
