29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation. At the inspection site, for calendar years 2009 through 2011, various errors and inconsistencies on the OSHA Form 300 Log materially impaired understandability of the nature of hazards, injuries and illnesses in the workplace. a) For calendar year 2009, case# 400476 did not specify the object or substance that directly injured or made the person ill, and the injury column (M) had both Injury (M1) and All other illnesses (M6) checked when the instructions state to check one. Case# 396414 did not specify the object or substance that directly injured or made the person ill, or the injury. Also, the case was missing the classification (cloumns G through J). And the page totals at the bottom were not filled in. b) For calendar year 2010, case# 56442 was missing the location of the event (column E). All four (4) cases were missing either the injury or illness, the parts of body affected, or the object or substance that directly injured or made the person ill in column F. Also, columns M1 through M6 were not completed for cases# 55880 and 56442. c) For calendar year 2011, all seven (7) cases were missing either the injury or illness, the parts of body affected, or the object or substance that directly injured or made the person ill in column F. Also, the page total for On job transfer or restriction (column L) had 4 instead of 135 (should be total of days and not cases). Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the OSHA 300 Log is being filled out completely and accurately.
