North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an unsecured hoistway door unlocking device that is to be accessible to authorized and emergency personnel only: a)facility - bakery freight elevator, for employees exposed to a moving elevator car in that the operating means for unlocking the door (chain) was not locked and secured to prevent opening of the outer elevators doors. Reference ASME A17.1-1996 - Safety Code for Elevators and Escalators section 111.6b Among other methods, one feasible and acceptable abatement method to correct this hazard is: 1) repair the existing locking covers over the chain.
