29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Location: Loading dock On or about 11/10/25, the employer did not ensure that employees operating stand-up powered industrial trucks had completed training and evaluation specified in paragraph (l) prior to operation. An operator on foot was struck by a stand-up powered industrial truck that was within three truck lengths. The operator of the striking truck had not completed their initial powered industrial truck evaluation prior to operating. BJ's Wholesale Club was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 1561080, citation number 1, item number 1 and was affirmed as a final order on 5/19/2022, with respect to a workplace located at 1260 East Woodland Avenue, Springfield, PA 19064.
