Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to cumulative trauma disorders: EMPLOYEES IN THE FOLLOWING DEPARTMENTS WERE REQUIRED TO PERFORM DUTIES WHICH WOULD REQUIRE REPETITIVE HAND, WRIST, ARM, NECK AND/OR SHOULDER MOTIONS RESULTING IN STRESSES LIKELY TO CAUSE UPPER EXTREMITY CUMULATIVE TRAUMA DISORDERS. A. DEPARTMENT 01 1) THE OPERATORS AND/OR ASSEMBLERS OF THE THERIMAGE MACHINES, LINES #14, 15 AND 16, WERE EXPOSED TO UNDUE REPETITIVE MOTIONS OF FINGERS, HANDS AND FOOT/LEG RESULTING IN STRESSES TO FLEXOR TENDON OF WRISTS, HANDS, ARMS/ELBOWS AND FATIGUE IN LOWER BACK OR LEGS FROM LOADING/UNLOADING BOTTLES ONTO/FROM MOVING CONVEYORS, HAND- TURNING BOTTLES AROUND WHILE UPPER BODY TURNED TO SIDES AND FROM LONG STANDING ON THE FLOOR. SINCE THE ULTIMATE RESPONSIBILITY FOR HAZARD CORRECTION RESTS ON THE EMPLOYER, FEASIBLE AND USEFUL METHODS TO ADDRESS THE PROBLEM OR REDUCE THE HAZARD INCLUDE, BUT ARE NOT LIMITED TO: A) EVALUATE THE FEASIBILITY OF DEVICE(S) OF EQUIPMENT SO THAT THE OPERATORS WOULD NOT NEED TO MANUALLY LOAD/UNLOAD BOTTLES ONTO/FROM CONVEYORS. B) EVALUATE THE FEASIBILITY OF DEVICE(S) OR EQUIPMENT SO THAT THE OPERATORS WOULD NOT NEED TO MANUALLY TURN BOTTLES AROUND. C) TRAIN OPERATORS AND/OR USE ALTERNATE METHOD(S) TO PERFORM DUTIES TO HAVE LESS REPETITIVE MOTIONS TO THE SAME FINGERS/HANDS/LEGS. D) PROVIDE ANTI-FATIGUE FLOOR MATS COVERING THE ENTIRE OPERATORS' STATIONS. 2) THE BOTTLE LOADERS OF THE THERIMAGE MACHINES, LINES #14, 15 AND 16, WERE EXPOSED TO UNDUE REPETITIVE MOTIONS OF FINGERS, HANDS, LEGS, SHOULDERS AND NECKS RESULTING IN STRESSES TO FLEXOR TENDON OF WRISTS, EXTENSION OF ELBOWS/SHOUDLERS, FATIGUE IN LOWER BACK AND CONTACT STRESSES ON RIGHT HIPS AND FRONT SIDE OF BODY FROM PICKING UP AND LOADING BOTTLES FROM THE HOPPERS ONTO MOVING CONVEYORS WITH BOTH HANDS WHILE "RESTING" RIGHT HIPS AND FRONT PART OF BODY ON FRAMES OF THE CONVEYORS, AND FROM STANDING ON HARD FLOOR SURFACE AND LOOKING DOWN AT WORK WITH NECKS BENT DOWN. SINCE THE ULTIMATE RESPONSIBILITY FOR HAZARD CORRECTION RESTS ON THE EMPLOYER, FEASIBLE AND USEFUL METHODS TO ADDRESS THE PROBLEM OR REDUCE THE HAZARD INCLUDE, BUT ARE NOT LIMITED TO: A) REDESIGN OF WORK STATION, USE OF ERGONOMICALLY ADJUSTED AND SWIVEL CHAIRS AND/OR EQUIVALENT TO ELIMINATE BENDING NECKS DOWN AND TO HAVE HANDS/WRISTS/ELBOWS/ARMS/SHOULDERS IN REST AND NATURAL COMFORTABLE POSITIONS. B) REDESIGN OR RESPOSTION OR BING/HOPPERS TO ELIMINATE STRETCHING OUT ARMS TO MANUALLY "RAKE" BOTTLES IN THE BINS/HOPPERS. C) PAD EDGE OF CONVEYOR FRAME(S) AND/OR USE OF ALTERNATE METHOD(S) TO ELIMINATE CONTACT STRESS. D) EVALUATE THE FEASIBILITY OF DEVICE(S) OR EQUIPMENT TO ELIMINATE MANUAL BOTTLE LOADING. E) TRAIN EMPLOYEES AND/OR USE ALTERNATE METHOD(S) TO PERFORM DUTIES TO HAVE LESS REPETITIVE MOTIONS TO THE SAME HANDS/WRISTS/ARMS/LEGS. 3) THE PACKERS OF THE THERIMAGE MACHINES, LINES #14, 15 AND 16, WERE EXPOSED TO UNDUE REPETITIVE MOTIONS OF FINGERS, WRISTS, ELBOWS, SHOULDERS AND BACKS RESULTING IN STRESSES TO FLEXOR TENDON OF WRISTS AND FINGERS, EXTENSION OF SHOULDERS AND FATIGUE IN LOWER BACKS AND NECKS FROM PICKING UP AND TURNING BOTTLES SIDEWAYS FOR DEFECTS, TWISTING UPPER BODY TO LEFT SIDE TO PUT BOTTLES IN THE CARDBOARD BOXES, BENDING BACK FORWARD WITH NECK BENT DOWN AND LONG REACHING DOWN ARMS/SHOULDERS WHEN STARTING NEW BOXES AND FROM TURNING UPPER BODY TO RIGHT SIDE FOR LAYERS. SINCE THE ULTIMATE RESPONSIBILITY FOR HAZARD CORRECTION RESTS ON THE EMPLOYER, FEASIBLE AND USEFUL METHODS TO ADDRESS THE PROBLEM OR REDUCE THE HAZARD INCLUDE, BUT ARE NOT LIMITED TO: A) REDESIGN OR REPOSITION OF WORK STATION TO ELIMINATE TURNING UPPER BODY SIDEWAYS AND BENDING BACKS FORWARD WITH NECK BENT DOWN TO LOAD AND PUT BOTTELS IN LAYERS. D) TRAIN EMPLOYEES AND/OR USE OF ALTERNATE METHOD(S) TO PERFORM DUTIES TO HAVE LESS REPETITIVE MOTIONS TO THE SAME HANDS/ELBOWS/BACKS. 4) THE FLASHING TRIMMING OPERATORS OF THE MOLDING MACHINES WERE EXPOSED TO UNDUE REPETITIVE MOTIONS OF HANDS AND ARMS RESULTING IN STRESSES TO FLEXOR TENDON OF FINGERS AND WRISTS, EXTENSION OF ELBOWS/SHOULDERS AND FATIGUE IN LOWER BACK AND NECK FROM LOOKING DOWN AT WORK WITH NECK BENT DOWN, PUTTING TRIMMED JARS INTO BOXES ON THE TABLE/SCALE WITH ELEVATED AND EXTENDED ELBOWS/SHOULDERS, TRIMMING OF FLASHING WITH THE OTHER HAND ELEVATED IN THE AIR OR RESTED ON THE LAP/THIGH WITH BENT ELBOWS ABOUT 90 DEGREES, CHECKING OUT THE MOUTH OF JARS WITH FINGERS FOR FLASHING WITH TWISTED WRISTS, TRANSFERRING JARS TO THE OTHER HAND, AND FROM PICKING UP JARS WITH THE RIGHT HAND WHILE HOLDING A SMALL KNIFE WITHIN THE SAME HAND/PALM WITH THE UPPER BODY TURN TO THE RIGHT SIDE.
