29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a)Fruit Room - Employees did not isolate the Maxwell Chase Tomato Slicer, Model MCT25 from all energy sources, when unjamming tomato trays from the machine, exposing the employee to an ingoing nip point. On 3/14/25 an employee sustained an amputation of the tip of his left ring finger while clearing a jam. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
