29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 01/21/2025 in the New Production Room 4, where an employee reached into the sprocket wheels and chains of the SANKO machine Line #73 to clean excessive sugar dust while the machine was running, the employee was exposed to the nip point between the sprocket wheels and chains. b) On or about 01/23/2025 in the New Production Room 4, where employees performed weekly cleaning, unjamming, and setup of the SANKO machines, requiring the removal of guards and disassembly of parts without locking out or controlling energy sources, employees were exposed to caught-by hazards in the event of unexpected machine start-up while reaching inside to remove components. ABATEMENT DOCUMENTATION REQUIRED The American Sugar Refining, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1597531, citation number 1, item number 1 and was affirmed as a final order on 09/09/2022, with respect to a workplace located at 692 Bailey Avenue, BUFFALO, NY 14206.
