29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29CFR 1910.147(c)(4)(ii)[A], 29CFR 1910.147(c)(4)(ii)[B], 29CFR 1910.147(c)(4)(ii)[C], and 29CFR 1910.147(c)(4)(ii)[D]: At a worksite located at 14201 Botts Rd, Grandview, Missouri. Employees were exposed to serious amputation, crushing and struck-by injuries, in that the employer had not developed specific energy control procedures for the shutting down and return operations for the various types of machinery, including but not limited to the Kubota Brabender DDW-FW40MS5 FW40/5 Plus-50 ISC Hopper, Serial number W022308/7 known to the company as the Twin Screw hopper. , which had multiple energy sources including pneumatic and electrical.
